Hair Transplant - Dr. Woods: Repair of Repair of Pluggy, Uneven and Scarred Hairline with FUE. This is a repair case with 2000 FUE grafts. Dr. Woods soften the pluggy, uneven and scarred hairline of this patient after 3 sessions. The repair was done by utilizing nape hair, chest and abdominal hair. None of the prior pluggy grafts were removed in the process. You can also see the chest donor pictures in this discussion.

Hair Transplant - Dr. Cole: Body Hair Transplant Megasession 5000 Grafts on the Crown Grown Out Results plus Amazing Leg Hair to Anterior Temples 2.5 Months Results. This is the first body hair transplant megasession grown out results posted in the forum. The patient had about 5000 BHT grafts placed into 80 square cm of bald crown. The result is exceptionally natural with very respectable yield. This discussion also features an attempt to lower the patient’s hairline with about 1700 grafts of leg hair as well as a 2.5 month post op results from leg hair transplanted into the anterior temples.

Hair Transplant - Dr. Woods: 5,500 FUE/BHT grafts, 4 Weeks Results, Grafts Started Growing Immediately Post Op . 4 weeks post op photos of a Norwood 6 patient who had received 2,500 scalp donor hair and 3,000 chest and abdominal/loin hair. Total of 5,500 grafts. The last session was 10 days ago to the frontal hairline. The scabs were removed immediately prior to these photographs being taken, hence the minor pinkness to the frontal hairline. Abdominal hair was placed to the crown 2 weeks ago prior to these photographs. A close up shot is provided and some of the scabs are still present. There is good growth right from the start. All graft hair shafts were 1 to 2 mm in length. As is clearly seen, many hair have extended by several millimeters since the procedure.

At that time Dr. Cole was one of a small group of hair restoration surgeons performing total microscopic follicular unit hair transplantation. Hair Transplant - Dr. Arvind: FUE Donor Sites, Up Close & Personal Using Donor Sealant Technique.
This discussion focuses on the appearance of the donor areas where Follicular Unit Extraction (FUE) has been performed and the Donor Sealant protocol has been followed.  Different scenarios have been presented:

1. the donor area has been shaved (hair being 1 mm or less)
2. the grafts/FUs have been densely extracted. 20-30 FUs have been extracted per sq cm.

The following issues are discussed:
1. Appearance, or otherwise, of hypopigmented dots.
2. Appearance, or otherwise, of empty spaces but no white dots.
3. Contrast with the hair density in the surrounding areas.
4. The distance from where any anomaly (dots, spaces, contrast) is visible and the distance at which it stops manifesting itself.

Hair Transplant - Dr. Cole: Results of 400 FIT/FUE grafts on an African-American patient. This patient’s pictures were taken one year after his initial procedure of only 400 FUE grafts into the temple areas. Here is a classic example of the “less is more” phenomenon and it also shows FIT/FUE results on a African-American patient. These grafts were extracted via Dr. Cole’s FIT/FUE method and, as usual, they have matured beautifully and healthily. This is a good example of what can be achieved when excellent hair characteristics meet ultra-refined surgical techniques. A former Norwood class 3a, this patient now enjoys a more flattering but still age-appropriate hairline.

This patient has some mini grafts or plugs in the temporal recession areas. Grafts in these areas should ideally be excised but the patient did not want to touch them at the moment. Dr. Arvind’s team, therefore, only concentrated on the front half of his scalp. Hair Transplant - Dr. Arvind: Repairing Minigrafts in the Temporal Areas with FUE Techniques. This is a patient who developed traction alopecia- loss of hair resulting from constant pulling and pressure exerted onto the scalp and hair.

He was previously administered rows of minigrafts and plugs in the front hairline and the temple areas by another doctor. Dismayed with the results, this patient convinced the original doctor to excise the plugs in the hairline. The plugs were excised in form of a strip which was then sutured closed. Hair Transplant - Dr. Arvind: How Donor Sealing Works in FUE Donor Sites? The reason behind the success of donor sealing is that 1. It returns the pigment bearing cells back to the donor sites where they are required to avoid appearance of 'white dots'.  2. It helps in promoting healing of the extraction sites by making "healing by primary intention" possible.

In the FUSE extraction sites, since the wounds are round and close and many in number, traditional methods of healing by primary intention are not possible (except in the donor exchange method). Therefore, the extraction site will heal by collagen formation from the dermis upwards. This collagen is white in color and is responsible for the "white dots".

Donor sealing creates a virtual seal of pigment bearing epidermis on top of each of the extraction sites. The surrounding epidermis, joins the donor sealant to provide "healing by primary intention".

The deeper dermal layers do heal by secondary intention and do form collagen, but the pigmented epidermis on top of the extraction site makes sure that the collagen tissue remains below and is not visible.

This is a 900 graft session of CIT (FUE), the Cole Isolation Technique, to the top mid-section of the scalp. The hairline was not grafted. Rather, Dr. Cole began removing several large, "pluggy" looking grafts from the right side of the hairline. The crown was not grafted either. Patient was quite satisfied with the healing of the right frontal hair line without scarring following the removal of several of the very large noticeable 2, 3, and 4 hair grafts that were on the hair line. The patient now returns to Dr. Cole to remove many other large grafts from the hairline and to build a new hairline that is softer and more natural. this will blend well with the outstanding coverage he obtained from his relatively small first procedure.

Hair Transplant - Dr. Cole: Vitiligo-Thoughts on Pigmentation in Strip Scars. Here is an interesting study to treat vitiligo. Vitiligo is a loss of skin pigment. Dark skin turns white in vitiigo. When you do a strip surgery, you find that the scar is often bright pink or white. One way we have treated these white strip scars in the past is to place grafts from FIT into the scar. We have found that this often helps to make the scars regain some pigment through the production of melanin. In this study normal skin grafts were placed into the white areas of skin. They also developed pigment and turned a more natural state of brown in this dark skinned individual.

With strip, there is the risk of the scar falling out of the safe zone- especially if it is the 3rd mega-strip and the doctor does not take into account the excised scalp tissue while calculating the safe zone. Hair Transplant - Dr. Cole: FUE and Repair Procedure of Over 3900 grafts from the Hairline throughout the Top and the Crown. This is a patient who had multiple plug grafts placed several years ago by the old punch graft method that left shotgun scars in the back of the scalp. He was a class 5A to 6 and all his hair from the top to the crown area was completely bald. He also has numerous elliptical scars in the back of the scalp where someone shamelessly talked this person into suturing a hairpiece to the top of his scalp. 13 months ago he underwent a FIT/FUE procedure of over 3900 grafts from the hairline throughout the top and including the crown. The old plug scars in the donor area reduced the total follicular density so that Dr. Cole’s team had to harvest follicular units between, above, and below the shotgun scars. The old hairline was too low and the hair growth angle was incorrect and unnatural. All of these factors required a complex repair plan that was carried out 13 months ago at Dr. Cole’s clinic.  Dr. Cole and his team also added some FIT grafts on the "shot gun scars" in his donor area.  The plug scars and other scars are not visible anymore.

Hair Transplant - Dr. Cole: Repair Case for Patient with Low Follicular Density Made Possible by FUE .This patient is a repair case who desired additional coverage in the crown. He had numerous procedures in the 1980s including plugs and scalp reductions. This left him with a pluggy hairline and pluggy tear drop shaped crown. Tear drop shaped crowns result from midline scalp reductions that lower the crown into the donor area and elevate it toward the front. He did not want to soften the hairline. He simply wanted more hair in the crown. His donor area was extremely tight and diffusely scarred with a low donor density. Dr. Cole and his team attempted one strip procedure on him, but this yielded only 400 grafts due to the low follicular density, excessive scarring, and extremely tight donor area. Subsequently, Dr. Cole was able to remove 2000 grafts with the FIT/FUE technique. This is 7 months post op results.
